Assess the psychosocial status of patients related to patient's terminal illness Provides assessment in patient's residence and carries out social evaluations and plan interventions Participate in the development of the individualized plan of care Educates patients and families on preparation of advanced directives Assist physician in understanding significant social and emotional factors Participate in quality assessment performance improvement team Counsel patient and family/caregivers with identified coping difficulties Serve as liaison between patients and community agencies Assists with planning for funeral arrangements, financial, legal, and health care decisions Maintain and develop contracts with public and private agencies What does a Home Health Social Worker do? Assess the psychosocial status of patients related to patient's illness Develop and provide multiple community and governmental resources to support a patient in their home environment Provides assessment in patient's residence and carries out social evaluations and plan interventions Participate in the development of the individualized plan of care Assist physician in understanding significant social and emotional factors Participate in quality assessment performance improvement team Serve as liaison between patients and community agencies Maintain and develop contracts with public and private agencies
Qualifications:
