Assess the psychosocial status of patients related to patient's terminal illness Provides assessment in patient's residence and carries out social evaluations and plan interventions Participate in the development of the individualized plan of care Educates patients and families on preparation of advanced directives Assist physician in understanding significant social and emotional factors Participate in quality assessment performance improvement team Counsel patient and family/caregivers with identified coping difficulties Serve as liaison between patients and community agencies Assists with planning for funeral arrangements, financial, legal, and health care decisions Maintain and develop contracts with public and private agencies What does a Home Health Social Worker do? Assess the psychosocial status of patients related to patient's illness Develop and provide multiple community and governmental resources to support a patient in their home environment Provides assessment in patient's residence and carries out social evaluations and plan interventions Participate in the development of the individualized plan of care Assist physician in understanding significant social and emotional factors Participate in quality assessment performance improvement team Serve as liaison between patients and community agencies Maintain and develop contracts with public and private agencies
Qualifications:
A graduate of an accredited master's program in social work Must possess an MSW or LCSW license in Oregon State Minimum of one year's experience in health care Experience with home health or hospice strongly preferred, but not required Possesses and maintains current BLS Certification A licensed driver with an insured automobile To learn more, call, or text our in-house recruiter Marta @ 425-320-9683 Live out of state?
